Even though not every single detail fits exactly, it looks very much like a Fiat 850 Spider with a hard top. The overall shape looks very similar, as well as many details. The bumpers obviously have been removed (a common procedure amongst race cars at that time). Maybe some sort of special version? Or simply movie altered.

I also think it was influenced by Alfa Spider, but final result can be too far from the look of the original car, to be listed under its name.
(If this what happens above the door handle is just a shadow and dirt and not a "Coke bottle design" then it is "less far".)

"From the very beginning of this project, John Lasseter had it in his mind to have the eyes be in the windshield. For one thing, it separates our characters from the more common approach where you have little cartoon eyes in the headlights. For another, he thought that having the eyes down near the mouth at the front end of the car made the character feel more like a snake. With the eyes set in the windshield, the point of view is more human-like, and made it feel like the whole car could be involved in the animation of the character."

Confirmed by storyboards of American Car, the original 2000 pitch for the film (listed everywhere else as 1999 though Lasseter stated he did not get the idea for the story until after a family road trip in 2000)
