1995-2005 body styling but I don't remember if they kept that single headlight style all the way through production. |

they did on the base models until about 97 or 98 because my father's 97 has single headlights. The cargo vans had them for even longer |

2003-2005 model by the 6 lug wheels in the first thumbnail. You can also see the face-lifted center caps in the main pic. The pre-2003 were black or a darker color and sat more flush with the wheel which would make it near impossible to see at that angle. |
