This is actually the E-Super Duty 1997-1998 or 1999-2002 E-450. Note the rear wheels. See how the rear wheel matches up with the front wheel. If the second wheel was still there it would sticking out further than the front wheel indicating the 4.56 dana rear axle; which was not offered on the E-350 chassis(4.10 was the axle ratio). Close but McCoy Miller actually produced both the E-350 and E-450 with a very similar box. Here are some examples.
