They don't mention this name in history chapter on A-F official site: http://autoform.pl/o-nas/historia-firmy/ (on the other hand it is not too detailed ). For example here Auto-Form cars are listed simply as "Polonez Caro (Plus) Ambulans". Like for now with B08 code of usual ambulances, although in case of Auto-Form conversion it didn't have to be valid.
