Some sources say it's based on Continental Mark IV, another listing claims it was based on the T-Bird, and that allegedly these were designed by George Barris.

Not sure if this is the exact one as the one that belonged to Sammy Davis Jr (apparently 12 were made... or 18 depending on which source to believe), but this might be the same:

According to Barris - Cars of the Stars by David Fetherston and George Barris it's the Lincoln and a contemporary magazine article states Barris Kustoms intended to build up to 22 a year. Various production figures are being rumoured across the web, but I reckon that only Barris Kustom Industries know how many were actually built.

A close relative is the more famous Bugazzi 74 by Barris Kustoms, another creation that was more or less inspired by the revival cars offered by Stutz Motor Car of America.
