Twin headlights have nothing to do with US specifically. Yes, for the US they were mandatory equipment because of the sealed beam headlamp regulation there, but they could be ordered on any W108/109 in any country and were automatically installed if you checked the 'halogen' headlamp option on your order sheet. More cars were delivered with them than with the 'regular' (thus actually rarer) single unit headlamps, which were still Bilux. Later 109s came standard with the 'export' lights, on W108s they remained an extra cost option, except for USA, where they always were mandatory.
In fact, many 'standard' headlight cars were later converted to 'export' lights by their owners, because they were
a.) cheaper to buy than standard ones in case those were broken,
b.) gave better light, and
c.) were considered 'cool'.
I still have a set of standard lights from when I did it. Should post them on ebay one of these days...

From this picture it is impossible to tell if the car is a W108 or W109, so I'd stick with IMCDB's common practice to list the base version then, which would make it a 250 S [W108].

I'm not saying you are wrong, it could be a 6.3. But we can't tell for sure from the pic.
