This is a 1958 GMC TDH-5106 "Old Look." According to OMOT, it was originally purchased for Fifth Avenue Coach Lines. In 1962, that company went bankrupt and a city entity called Manhattan and Bronx Surface Transit Operating Authority, a subsidiary of New York City Transit Authority, took over their operations.
