It's this one: http://picturecarwarehouse.net/cars/5576/gmc/swat-stepvan/ The extra latches(?) on the bonnet and bit of missing paint behind the door match it. PCW call it a 2004 GMC but the plate 8M55491 is a valid plate from a 1995 GMC P-3500.
