On the ealy 1980's versions the lower cost models had only 2 headlamps and the indicators mounted below while higher spec models had 4 headlamps and the indicators mounted in the bumper. The parts can easily be switched/swapped and that is probably what happened here.

Maybe its mix n match, certainly looks strange. So far Ive found that it has a 1982 Scottsdale headlight setup with a 1985/86 Silverado grill setup. heres the 82 headlight linknote the elongated egg crate grill lines below the headlights. Heres the1985/86 grill center component linkThe bumper is also from 1982........So Im going to say I believe its a 1982 Chevrolet Scottsdale Suburban.

All 1982-1982 Chevy full-size trucks and SUV's had the turn signals in the bumper. I have to admit though, I too haven't seen too many like this. They must not have been a big seller in those two years.
