was the US the only place that got these named as both 323 and Protegé? For Instance I had 2 Protege DX sedans one was a 1990 and was badged "323 Protegé" and the other was a 1991 and badged simply as "Protegé" other than the badge the cars were identical.

It was a 323 here.

However, an earlier model was built in South Africa after Mazda were finished with it and was called a Sao Penza. Those were imported into the UK and their used to be two in the small town where I live.

One of which had a tap (faucet or spigot to our US correspondents) instead of a gear knob. I have no idea if that was a standard thing but I would assume not.

Are there any SAO Penzas on the base?

Not under that name, but it's been mentioned in comments on cars in South African films listed as 323s and Ford Tracers.

Those Sao Penza's are uber rare. Never seen one in the flesh, and their is only one picture of them in Google:

Somewhere I have pictures of the one that used to live around the corner from me. The only one of those pictures I can find of it though is the one with me aged six in the immediate foreground and a very blurred Penza in the background.

They were rare to start with but by now with most of them aged nearly 20 years old they must be virtually extinct.
